Immunohistochemistry (IHC): The Test That Tells Your Doctors What Kind of Cancer You Have

Immunohistochemistry — usually shortened to IHC — is an extra test the pathologist runs on your biopsy sample. It uses special stains that attach to specific proteins in the cells, lighting them up so the pathologist can see them. This helps answer questions a basic look under the microscope can't: exactly what type of cancer it is, where it started, and sometimes which treatments are likely to work. It's a major reason some reports take extra days — and that extra time usually means a more precise answer.

If your pathology report mentions "IHC," "immunostains," or "special stains," and you've been left wondering whether that's something to worry about — it isn't. Quite the opposite. IHC is one of the tools that makes a modern cancer diagnosis precise. This guide explains what it is, in plain words.

A quick note first: this explains the test in general terms. What your specific IHC results mean for you is for your oncologist to interpret alongside the rest of your case. Please don't draw conclusions from individual markers on your own.

01 — The test

What IHC actually is

When a pathologist first looks at your biopsy under the microscope, they can often tell that cancer is present and get a general sense of its type. But sometimes the cells alone don't tell the full story — two very different cancers can look similar at first glance.

IHC solves this. The lab applies special stains, each designed to stick to one particular protein that may be on or inside the cells. If that protein is present, the stain "lights it up" — the pathologist sees a colour change under the microscope. If it's absent, no colour appears.

By testing for a panel of these proteins, the pathologist builds a kind of fingerprint of your cancer.

What IHC tells your doctors

Those protein patterns answer several important questions:

What type of cancer is this, exactly?

Two cancers that look alike under the microscope can have completely different protein fingerprints — and therefore different diagnoses and treatments.

Where did it start?

This matters most when cancer is found in one place but may have originated elsewhere (for example, cancer found in a lymph node that began in another organ). IHC can point back to the likely origin.

Are there features that guide treatment?

Some IHC markers directly influence treatment choices. (In breast cancer, for instance, the ER, PR, and HER2 markers — which we'll cover in a dedicated guide — come from this kind of testing.)

In short, IHC turns "this is cancer" into "this is which cancer, from where, with these characteristics" — the detail your treatment plan depends on.

03 — The wait

Why it can make your report take longer

A basic microscopic examination can be done relatively quickly. IHC adds steps: selecting the right panel of stains, applying them, letting them develop, and interpreting the results. That's often several extra days.

It's natural to find the wait stressful — but it's worth reframing. Those extra days usually mean your pathologist is being thorough, not slow. 04 — A common confusion

Is IHC the same as genetic testing?

This is one of the most common points of confusion, so it's worth being clear: no, IHC is not genetic testing.

IHC reads proteins; genetic testing reads DNA. Complementary, not interchangeable. — **Two tests, two layers of the cell.** IHC reads the proteins a cell is making; genetic testing reads the DNA inside it. They're complementary, not interchangeable — one doesn't imply the other.

They're complementary — many cancers benefit from both — but they are different tests looking at different layers of the cell. If your report mentions IHC, that doesn't necessarily mean genetic testing has been done, and vice versa.

IHC builds a fingerprint. Special stains attach to proteins and light them up — turning "this is cancer" into "which cancer, from where."
A delay for IHC usually means precision, not bad news. The extra days are your pathologist being thorough.

05 — One caution

What you should not conclude on your own

A single IHC marker, read in isolation, can be misleading. These results only make sense as a pattern, interpreted together and in the context of your cancer type, your scans, and your overall picture — which is exactly what your pathologist and oncologist do. A marker being "positive" or "negative" isn't good or bad on its own; it's a piece of information that means different things in different cancers. Bring your report to your oncologist with any questions marked.

Frequently asked questions

Is IHC the same as genetic testing?

No. IHC tests for proteins the cells are making; genetic (molecular) testing examines DNA and genes. They answer different questions, are usually done by different methods, and many cancers benefit from both. One doesn't replace the other.

Does my cancer being "positive" for a marker mean it's worse?

Not necessarily. A positive or negative marker isn't inherently good or bad — it depends entirely on the marker and the cancer type. Some positive markers actually open up more treatment options. Your oncologist interprets what each result means for you.

Why does IHC take extra days?

The lab has to select the right stains, apply and develop them, and interpret the results carefully. Those extra days usually reflect thoroughness, and lead to a more precise diagnosis.

Is IHC done on the same sample as my biopsy?

Usually yes. In most cases IHC is performed on the same tissue already taken during your biopsy, so it doesn't require a new procedure.
